We take your raw, unorganized medical records, sort through and organize them chronologically, delineate the clinically significant information, and summarize that information into S.O.A.P. format.

Receive the completed Summary as a Word document and PDF in 3-5 business days, either in an email or directly into your personal Dropbox folder. Expedited service is available (see Rates for details)!

Our team has the clinical knowledge and expertise to identify all significant data such as diagnostic tests, lab results, DEA profiles, medications, diagnoses, surgical procedures, therapy notes, and so on.

Original documents can be kept in your private Dropbox folder or deleted/destroyed.
